Athletics

All students who participate in athletics are required to complete several forms.

1. Rank One: All Rank One Sport online forms must be completed and physical turned in before participation in any sport or off season. View Rank One Instructions
    
2. Physical: You cannot fill out the physical online; it must be hand delivered to your Coach or Athletic Director’s office. You can also scan and email completed physical form to yohancebrown@dcsok.org.
   • Athletic physicals expire on June 30.
   • A current OSSAA physical form is required before any participation in a club, sport and/or in-school sport.
   • Only physicals dated after May 1, 2024 can be accepted.
